MaineHealth Care at Home is a leading provider of comprehensive home health, hospice, and population health services in the Southern and MidCoast regions on Maine.
As a 501(c)(3) organization and member of MaineHealth, we are committed to our not-for-profit mission of “Helping people to be as healthy and independent as possible, at home and in the community” by delivering skilled and compassionate services to patients, regardless of their ability to pay.

OUR SERVICES:
At MaineHealth Care at Home, we are proud to support patients through every stage of life.
Nursing
With their advanced clinical skills and expertise, our nurses provide assessment, teaching, and direct care to patients in need of support to manage/recover from serious illness and regain independence. Services are supported by experienced home health aides who support the patient experience.
Specialties include chronic disease management, complex wounds, cardiac and pulmonary care.
Additionally, we offer a robust telehealth service that utilizes a tablet, dedicated peripherals, and specially-trained nurses to support the care of patients through skilled telehealth care plans.
Rehabilitative Therapies
Our therapists use the latest treatment techniques to help patients gain independence and strength through education, exercise, and supportive devices.
We offer physical therapy, occupational therapy, and speech-language pathology to deliver rehabilitation from orthopedic surgery, cardio-pulmonary surgery/disease, and post-stroke care.
Hospice
In the MidCoast, we provide a dedicated hospice service to patients facing life-limiting illness. We offer a support system of medical, social, emotional, and spiritual resources to promote dignity and affirm quality of life for the patient and their family.
Services are provided in the home as well as at the Sussman House, our in-patient hospice house located on the campus of Pen Bay Medical Center in Rockport.
Population Health
We believe that education and preventative care are the foundation of good health and disease management. Our population health nurses provide health screenings, education, immunizations, and foot care at clinics and public sites throughout our service area.
Lifeline
MaineHealth Care at Home partners with Philips Lifeline to offer a range of alert devices that help individuals maintain safety and independence. These devices offer connectivity to emergency resources 24 hours a day, 365 days a year.
